directly from the city, county or state where the event occurred by
downloading the free order forms.

Vital Records 1923-Present - The fee for one certified
copy is $15.00. The fee for additional certified copies of the same record is $6.00
each. Send self-addressed stamped envelope with your payment.

All requests are filled within 7 - 10 working days after receipt. See below for
search request requirements.
This includes a five-year search for the requested record. The fee is non-refundable
whether the record is found or not. It is important to provide as much information
as possible when writing in with a request. For example, with birth records, it
is extremely helpful to include the father's name.

• Maine State Birth Certificate By Mail To fulfill your request, the following information
is needed:
Name on the birth record.
Date of birth.
Name of city/town the birth occurred in.
The names of the parents - please use the mother's
maiden name.
State your relationship to the name on the birth
record being requested.
Please include a daytime telephone number.
Please make the check payable to: "Treasurer -
State of Maine".
